Federal safety experts have fined Spray Polyurethane Foam Company after a deadly work site accident in the Mission Valley.
Last August, a roofer died when he fell 30 feet in the 9600 block of Plaza Circle.
An investigation showed the building was not strong enough to support the worker's weight.
OSHA penalized the company $8,700.
